Tier 2 — Medical Emergency Response Plan

Sri Lanka Medical Travel Assessment

Double Lung Transplant Recipient — In-Country Evaluation

RECOMMENDATION: DO NOT TRAVEL — RISK TOO HIGH

Advisory Source

Dr. Tolusha Harischandra

First female cardiac surgeon in Sri Lanka • Founder of Sri Lanka's sole ECMO program

Dr. Harischandra completed a fellowship at Glenfield General Hospital in Leicester, UK, and returned to build the nation's only extracorporeal membrane oxygenation (ECMO) program at National Hospital Galle. Despite severe economic pressures to practice abroad, she has remained dedicated to serving the people of Sri Lanka. Her assessment was delivered in person during on-the-ground evaluation of Sri Lankan healthcare infrastructure.

There is arguably no one on the island more experienced or qualified to evaluate whether a two-time double lung transplant recipient can safely travel to Sri Lanka. Her recommendation, fully concurred with by the on-site evaluator, is unequivocal: do not travel.

Primary Risk Factors

Critical

No On-Island Post-Transplant Expertise

Should a significant medical event occur, there is no physician on the island with the knowledge and experience to properly manage a two-time double lung transplant recipient. This is the single most disqualifying factor.

Critical

Dengue Fever — Endemic, Vaccine Contraindicated

Dengue is endemic to Sri Lanka. The available vaccine is a live-attenuated virus, which is contraindicated for immunocompromised patients. There is no viable prophylaxis beyond mosquito avoidance.

High

Fungal & Opportunistic Infection Environment

The tropical climate and healthcare environment carry elevated fungal and opportunistic infection risks, which are particularly dangerous under chronic immunosuppression with tacrolimus.

Historical Precedent

Sri Lanka's First Thoracic Organ Transplant: A combined heart-lung transplant was performed on a local physician with a visiting Indian surgeon. The patient never left the ICU, dying from a fungal infection. This outcome underscores the infection risk and the limitations of post-operative care infrastructure on the island.

Contingency Plan (If Travel Proceeds Against Advice)

Note: This contingency exists only as a last-resort framework. Traveling against the stated medical advice is strongly discouraged.
  1. Immediate Stabilization

Proceed to National Hospital Galle, where Dr. Harischandra runs the ECMO program, for emergency stabilization and assessment.

  2. Air Ambulance Evacuation

Arrange air ambulance transport to Chennai, India, which maintains a robust and established lung transplant program.

  3. Definitive Care in Chennai

If required, Aksanio can establish relationships with the receiving transplant team in Chennai to ensure continuity of care upon arrival.

Facility Role Capability
National Hospital Galle Emergency stabilization ECMO program (Dr. Harischandra)
Chennai transplant center Definitive treatment Full lung transplant program — Aksanio ground coordination available

Logistical Items

IV Fluid Administration — Arrival Protocol

Three options are available for receiving intravenous fluids immediately upon arrival at Bandaranaike International Airport (CMB), listed in order of speed from landing to infusion. A fourth self-administration option is also noted.

  1. Airport Medical Center (Fastest)

After collecting checked baggage, go to the airport help desk near the baggage claim area and request medical assistance — the clinic is not a walk-in facility. The help desk will summon the on-duty physician from the Airport Medical Center. A full-time doctor is present with qualified nursing staff and beds available. IV fluids can be administered on-site before exiting the airport.

  2. Lanka Hospitals — Walk-In

Proceed directly from the airport to Lanka Hospitals PLC. IV fluid administration is available as a walk-in patient with no prior appointment needed.

  3. Lanka Hospitals — Mobile Home Care Unit

Go to accommodation first and call Lanka Hospitals to dispatch their mobile home care unit directly to her location. The team will travel to the accommodation and administer IV fluids on-site.

    Lanka Hospitals: 011 5 430 000

Self-Administration Option: If she can fully self-administer IV fluids, this can be done independently at the accommodation. Lactated Ringer's and Normal Saline are available over the counter at any pharmacy in Sri Lanka — no prescription required.

Laboratory Testing — Tacrolimus Levels & CMV Viral Load

Should travel proceed against advice, monitoring tacrolimus trough levels and CMV viral load will be essential. Four facilities in the Colombo area can perform these tests. Pricing, turnaround times, accreditation, and payment methods are detailed below.

Lanka Hospitals PLC
CAP Accredited JCI Accredited Recommended
Tacrolimus Level: Rs. 6,500 — same-day results
CMV Viral Load: Rs. 6,750 — results within 7 days
Accreditation: College of American Pathologists (CAP); JCI Accreditation
Payment: Credit card accepted (confirmed)
Best overall option: Most competitive pricing, same-day tacrolimus results, shortest CMV turnaround (7 days), and the highest accreditation tier (CAP + JCI) of any facility on the island.
Nawaloka Hospital
SLAB / ISO 15189:2022

Lanka Hospitals Rs. 6,500 Same day Rs. 6,750 7 days CAP + JCI
Nawaloka Rs. 16,200 Same day Rs. 19,200 10 days SLAB / ISO 15189:2022
SJGH (Kotte) Rs. 7,500 1 day ~Rs. 8,000 3–7 days SLAB / ISO 15189

Recommendation: Lanka Hospitals PLC offers the best combination of competitive pricing, fastest turnaround (same-day tacrolimus, 7-day CMV), and highest accreditation (CAP + JCI). It is the most efficient choice for monitoring a post-transplant patient if travel proceeds.

Summary

Final Determination: Based on a three-day in-country assessment and the expert opinion of Dr. Tolusha Harischandra — the most qualified authority on the island — travel to Sri Lanka for a two-time double lung transplant recipient carries unacceptable risk. The absence of on-island transplant expertise, endemic infectious disease threats incompatible with immunosuppression, and the historical precedent of a fatal fungal infection following Sri Lanka's only thoracic organ transplant all converge on a clear conclusion: this trip should not proceed.
