Advisory Source
Dr. Tolusha Harischandra
First female cardiac surgeon in Sri Lanka • Founder of Sri Lanka's sole ECMO program
Dr. Harischandra completed a fellowship at Glenfield General Hospital in Leicester, UK, and returned to build the nation's only extracorporeal membrane oxygenation (ECMO) program at National Hospital Galle. Despite severe economic pressures to practice abroad, she has remained dedicated to serving the people of Sri Lanka. Her assessment was delivered in person during on-the-ground evaluation of Sri Lankan healthcare infrastructure.
There is arguably no one on the island more experienced or qualified to evaluate whether a two-time double lung transplant recipient can safely travel to Sri Lanka. Her recommendation, fully concurred with by the on-site evaluator, is unequivocal: do not travel.
Primary Risk Factors
No On-Island Post-Transplant Expertise
Should a significant medical event occur, there is no physician on the island with the knowledge and experience to properly manage a two-time double lung transplant recipient. This is the single most disqualifying factor.
Dengue Fever — Endemic, Vaccine Contraindicated
Dengue is endemic to Sri Lanka. The available vaccine is a live-attenuated virus, which is contraindicated for immunocompromised patients. There is no viable prophylaxis beyond mosquito avoidance.
Fungal & Opportunistic Infection Environment
The tropical climate and healthcare environment carry elevated fungal and opportunistic infection risks, which are particularly dangerous under chronic immunosuppression with tacrolimus.

IV Fluid Administration — Arrival Protocol
Three options are available for receiving intravenous fluids immediately upon arrival at Bandaranaike International Airport (CMB), listed in order of speed from landing to infusion. A fourth self-administration option is also noted.
-
Airport Medical Center (Fastest)
After collecting checked baggage, go to the airport help desk near the baggage claim area and request medical assistance — the clinic is not a walk-in facility. The help desk will summon the on-duty physician from the Airport Medical Center. A full-time doctor is present with qualified nursing staff and beds available. IV fluids can be administered on-site before exiting the airport.
-
Lanka Hospitals — Walk-In
Proceed directly from the airport to Lanka Hospitals PLC. IV fluid administration is available as a walk-in patient with no prior appointment needed.
-
Lanka Hospitals — Mobile Home Care Unit
Go to accommodation first and call Lanka Hospitals to dispatch their mobile home care unit directly to her location. The team will travel to the accommodation and administer IV fluids on-site.
